It is valid anywhere in the world except your home country. It covers injuries and illnesses.<\/p>\n<ul class=\"xoxo blogroll\">\n<li><strong>Your Insurance Website &amp; App\u00a0<\/strong>\u2013 Use the <a href=\"http:\/\/www.culturalinsurance.com\/mycisiapp\/\" target=\"_blank\" rel=\"noopener noreferrer\">myCISI app<\/a> and the <a href=\"https:\/\/www.mycisi.com\/CISIPortalWeb\/pub\/login.aspx?PT=GEN\" target=\"_blank\" rel=\"noopener noreferrer\">myCISI desktop log-in<\/a> to obtain information about your insurance coverage, account and reprint your insurance card.<\/li>\n<\/ul>\n<ul>\n<li><strong>Your Insurance Card<\/strong> \u2013 You should carry your personal health insurance card with you at all times. The card is attached to an email from \u2018My CISI\u2019 which you received at orientation. Search your email for \u201cenrollments@mycisi.com\u201d. \u00a0<strong>If you cannot find the email or you lose your insurance card, you can:<\/strong>\n<ul>\n<li style=\"list-style-type: none;\">\n<ul>\n<li>Go on the <a href=\"https:\/\/www.mycisi.com\/CISIPortalWeb\/pub\/login.aspx?PT=GEN\" target=\"_blank\" rel=\"noopener noreferrer\">myCISI desktop log-in<\/a>\u00a0and print a new card.<\/li>\n<li>Contact the insurance company by calling 203-399-5130 or emailing <a href=\"mailto:Claimhelp@culturalinsurance.com\">Claimhelp@culturalinsurance.com<\/a><b>. <\/b>Please include your au pair ID number in the email along with your host family\u2019s name and address. (If you call\u00a0you may wait a while on hold with no music or sound, this is normal.)<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<\/li>\n<li><strong>Doctor Search<\/strong>\u00a0&#8211;\u00a0You can search this database for an urgent care clinic or doctor who takes your insurance: <a href=\"https:\/\/www.aetna.com\/dsepublic\/#\/contentPage?page=providerSearchLanding&amp;site_id=passport\">click here<\/a> After you enter your zip code, you will be asked to Select a Plan, choose \u201cPassport to Healthcare Primary PPO Network\u201d)<\/li>\n<\/ul>\n<h2 class=\"widgettitle\"><span style=\"color: #008080;\">Social Security Card<\/span><\/h2>\n<p>Au pairs should wait until they have been in the U.S. for at least 4 days before going to apply for their Social Security card. You may go before 4 days, but please understand that you may encounter a problem. If an au pair\u2019s information is not yet in the database, the application cannot be processed at the time and will get put to the side and processed at a later date. This can lead to a much longer wait for the card to arrive in the mail.<\/p>\n<p><strong>Step 1 \u2013 Gather Your Documents<\/strong><\/p>\n<ul>\n<li style=\"list-style-type: none;\">\n<ul>\n<li>Print your <a title=\"You need to go here and print a paper copy of your I-94 card before going to SS Office or getting the MD license\" href=\"https:\/\/i94.cbp.dhs.gov\/I94\/#\/home\">I-94 Card<\/a>\u00a0(Click on \u201cGet Most Recent I-94\u201d at the top of the page)<\/li>\n<li>Print your Participation Letter (on your au pair portal)<\/li>\n<li>Complete the <a href=\"https:\/\/www.ssa.gov\/forms\/ss-5.pdf\">Social Security Card Application<\/a>\n<ul>\n<li><span style=\"color: #ff0000;\">Be sure to fill out the application listing your name EXACTLY as it is shown on your visa.<\/span> If you have more than one first or last name, it could be listed differently than how you would think.<\/li>\n<li>In box 5 for citizenship information, check off \u201cLegal Alien Allowed To Work\u201d.<\/li>\n<li>If there are any questions you are unsure about, leave them blank and ask at your appointment.<\/li>\n<\/ul>\n<\/li>\n<li>Take the following documents with you to the SS Office:\n<ul>\n<li>passport\/visa<\/li>\n<li>DS-2019<\/li>\n<li>country driver\u2019s license<\/li>\n<li>international driver\u2019s permit or certified translation<\/li>\n<li>participation letter<\/li>\n<li><a href=\"https:\/\/i94.cbp.dhs.gov\/I94\/#\/home\">I-94 card<\/a><\/li>\n<li>completed SS application form<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p><strong>Step 2 \u2013 Go to the Social Security Office<\/strong><\/p>\n<p>No appointments are needed. Social Security has resumed walk-in service at all locations.<\/p>\n<ul>\n<li>Go to the\u00a0<a href=\"http:\/\/www.socialsecurity.gov\/locator\/\">Social Security Office Locator<\/a>\u00a0to find an office near you.<\/li>\n<li>Social Security Offices tend to be busiest early in the month, early in the week, and early in the day. Avoiding those times should reduce your wait time.<\/li>\n<\/ul>\n<h2><span style=\"font-size: 1.285714rem; color: #008080;\">Travel Outside of the U.S.<\/span><\/h2>\n<p><span style=\"color: #ff0000;\">NOTE: Before planning any international travel, look at your visa (inside your passport). Check to see the expiration date printed on your actual visa. That expiration date printed on your visa is sometimes earlier than your actual end date. You must return at least 1-2 weeks prior to the earlier date.\u00a0<\/span><\/p>\n<p><strong>Travel Validation is Required to Re-enter the U.S.<\/strong>\u00a0\u2013 Complete the <a href=\"https:\/\/www.aupairinamerica.com\/pdf\/travel-validation-request-form.pdf\">Travel Validation Request Form<\/a> (To obtain APIA\u2019s signature on your DS-2019 before traveling outside of the U.S.)\u00a0<em>Note:<\/em>\u00a0<em>The travel validation ensures that you are in good standing with the program, it is not formal permission to travel.\u00a0<\/em><\/p>\n<ul>\n<li style=\"list-style-type: none;\">\n<ul>\n<li><span style=\"color: #ff0000;\"><strong>* NEW*<\/strong><\/span> You now have two options for sending the travel validation request form to Au Pair in America. The <a href=\"https:\/\/www.aupairinamerica.com\/pdf\/travel-validation-request-form.pdf\/\">form<\/a> explains the process in detail.\n<ul>\n<li><strong>Option 1<\/strong> \u2013 Email the scanned form along with your scanned DS-2019 form to APIA as .pdf files. The email address and complete instructions are on the form.<\/li>\n<li><strong>Option 2<\/strong> \u2013 In the regular mail or an expedited service, send the <a href=\"https:\/\/www.aupairinamerica.com\/pdf\/travel-validation-request-form.pdf\/\">form<\/a> along with your original DS-2019 to APIA, at least 3-4 weeks prior to international travel. The mailing address and complete instructions are on the form.\n<ul>\n<li>Check out <a href=\"https:\/\/blogs.aupairinamerica.com\/dcc\/how-to-address-an-envelope-in-the-usa-2\/\">this blog post<\/a> for step-by-step instructions on how to send mail in the U.S.<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p><strong style=\"font-size: 1.142857rem;\">Travel Rules by Program Year<\/strong><\/p>\n<ul>\n<li style=\"list-style-type: none;\">\n<ul>\n<li><strong>Year 1 Au Pairs<\/strong> must have a valid visa to travel internationally and plan to return to the U.S. at least 1-2 weeks prior to the expiration date printed on your visa. This can be different from the actual program end date.\n<ul>\n<li style=\"list-style-type: none;\">\n<ul>\n<li><a href=\"https:\/\/www.aupairinamerica.com\/pdf\/year-1-international-travel-checklist.pdf\">Year 1 International Travel Checklist<\/a><\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<\/li>\n<li><strong>Year 2 Au Pairs<\/strong> may visit Canada, Mexico, and the <a href=\"https:\/\/www.ice.gov\/sevis\/travel#which-islands-are-defined-as-adjacent-islands--2\">adjacent islands<\/a> due to the <a href=\"https:\/\/travel.state.gov\/content\/travel\/en\/us-visas\/visa-information-resources\/visa-expiration-date\/auto-revalidate.html\">Automatic Revalidation Policy<\/a>. (Carry this info with you for travel to Canada, Mexico, &amp; Caribbean travel in Year 2.)
